New patients who wish to register with the practice should collect a registration pack from the ground floor reception and complete the FP1 form immediately; we ask that you attend to register between the hours of 9.00am and 6.00pm Monday to Friday. An appointment will then be made with the health care assistant for a New Patient Medical.

Practice Registration Policy
This surgery has an 'open' patient list and there is a registration policy for any patient wishing to register. The practice registers without any discrimination, but does retain the right to refuse registration on other grounds if it is felt justified. The reasons for which will be provided in writing in that case.
Any patient wishing to register has to have a home address inside the practice’s catchment area (please see map on the wall at the reception desk or on this page).
Proof of the following documentation will be required as part of the registration process (Please note that it is essential that these documents are viewed before registration can take place): -
Proof of ID = Passport, UK drivers licence or birth certificate
Proof of address = Utility bill, bank statement, Tenancy agreement, benefits, Inland Revenue or official government letter (not hand written) that is not more than 3 months old.
Please note that a mobile telephone bill can not be used as proof of address.
